Dear Parents,

            

At outings and special events pictures are often taken of the activities and the students and adults participating.  At times we have the ability to put pictures in the newspaper to let the community know more of the program.  I have added a Photo Release Form to your packet to sign giving permission for photos to be used. 

If you would rather not have your child’s picture be in the newspaper/ program, than please sign at the bottom of the form to indicate that as well.  Thank you again for your time in reading and filling out the forms for your son/daughter to take part in Mentors Matter/ The Bridge.

       Photographs of your child, __________________________, and his/her Mentor may be taken at activities for promotional purposes.  These pictures may be used in program newsletters or newspaper articles.
